Job description

Street Health Outreach Medical Assistant

At Father Joe's Villages, Street Health meets people where they are - in the places most systems don't reach. Our Street Health team provides outreach, engagement, and integrated healthcare support for individuals experiencing homelessness across San Diego County, helping people improve health outcomes and take meaningful steps toward stability and housing.

As a Street Health Outreach Medical Assistant, you will play a unique hybrid role that combines field-based outreach, care coordination support, and clinical medical assistant duties. You will meet patients in the community, build trust over time, and connect individuals to the healthcare system while supporting ongoing care in both field and clinic settings.

What Success Looks Like

Success in this role means building trusted relationships with individuals who are often disconnected from care and helping them stay engaged in services over time.

You will work in dynamic environments - including streets, encampments, shelters, and clinics - supporting individuals with complex medical, behavioral health, and social needs. Your ability to combine clinical skills, outreach, and care coordination will directly improve patient engagement, continuity of care, and overall health outcomes. This is a strong fit for someone who is:

  • Passionate about serving individuals experiencing homelessness and complex health needs
  • Comfortable working in both field and clinic environments
  • Skilled at balancing clinical tasks with relationship-based outreach
  • Motivated by meaningful, day-to-day impact on people's lives

Your Role

  • Conduct street-based and community outreach to engage individuals experiencing homelessness and connect them to healthcare and support services.
  • Maintain an assigned caseload, including Enhanced Care Management (ECM) members, to support ongoing engagement, follow-up, and continuity of care
  • Provide clinical support including taking vital signs, documenting patient information, and assisting licensed clinicians with patient care activities.
  • Support care coordination across healthcare, housing, behavioral health, and social service systems in collaboration with interdisciplinary teams and community partners
  • Complete accurate, timely documentation for outreach, clinical services, and ECM-related activities in electronic health records

Desirable Candidate Profile

  • High School Diploma or GED required.
  • Medical Assistant Certificate required.
  • Minimum of one (1) year of experience in outreach, healthcare, case management, care coordination, or related direct service work with underserved populations.
  • Experience working with individuals experiencing homelessness, behavioral health conditions, substance use disorders, or complex health and social needs preferred.
  • Experience providing ECM services preferred.
  • Strong commitment to serving vulnerable populations with compassion, respect, and a belief in each person's capacity for stability and recovery.
  • Basic computer proficiency and ability to complete documentation in electronic systems.
  • Valid California driver's license.

Key Competencies

  • Knowledge of medical assistant practices and clinical support procedures.
  • Understanding of trauma-informed care, harm reduction, ECM, and whole-person care models
  • Strong documentation skills and comfort using electronic health records
  • Ability to manage an assigned caseload across field and clinic settings
  • Strong interpersonal skills with the ability to build trust and engage diverse populations
  • Ability to work independently in the field and collaboratively within multidisciplinary teams
  • Commitment to delivering services with compassion, respect, empathy, empowerment, and dignity

Work Schedule

This is a full-time position involving field-based outreach and clinic-based work. Duties are performed across streets, encampments, shelters, and clinical environments. Schedules are typically Monday through Friday; hours may vary based on outreach schedules, patient needs, and program priorities.

Compensation & Benefits

The anticipated starting pay range for this role is $24.00 to $27.61 per hour. Final placement and starting rate will be determined based on the candidate's experience, qualifications, certification status, and internal equity considerations.

Father Joe's Villages offers a comprehensive benefits package, including medical, dental, and vision coverage, retirement plan options, paid time off, and professional development opportunities. Employees also have access to wellness programs and employee assistance resources to support work-life balance and overall well-being.

Why Join Father Joe's Villages

Join an organization deeply rooted in compassion, respect, empathy, empowerment, and dignity. Make a meaningful impact by supporting individuals and families as they transition from homelessness to housing stability. Work alongside a collaborative, mission-driven team committed to holistic, trauma-informed care. Grow your skills in case management, housing stability, and client advocacy within a respected community organization.

Conditions of Employment

Employment is contingent upon successful completion of applicable pre- and post-employment requirements, which may include background checks, health screenings, license or certification verification, driving record review, and other requirements relevant to the role. These checks are conducted following a conditional offer of employment.

Equal Employment Opportunity

Father Joe's Villages is an equal opportunity employer dedicated to building a diverse, inclusive workforce. All qualified applicants will receive consideration without regard to race, color, religion, sex, sexual orientation, gender identity, age, national origin, disability, veteran status, or any other characteristic protected by law.
