Job Description
We are seeking a Licensed Independent Social Worker to join our team! You will assess and treat individuals with mental, emotional, or substance abuse problems, including abuse of alcohol, tobacco, and / or other drugs. Activities may include individual and group therapy, crisis intervention, case management, client advocacy, prevention, and education
Responsibilities :
- Assess nature of clients' situations by interviewing and reviewing personal history
- Carry caseload of 15-20 clients
- Develop and execute individual treatment plans
- Determine appropriate milestones to gauge client progress
- Communicate client updates to all relevant parties
- Facilitate referrals to other healthcare professionals and programs
- Maintain accurate client documentation
Qualifications :
Licensed Social Worker
At least 5years previous work experience in behavioral health care / social services.
