Position Overview :

As a Hospice Social Worker, you will provide psychosocial support to patients and their families, assisting them in coping with the challenges of terminal illness and end-of-life care.

This position involves assessing the social and emotional needs of patients and families, providing counseling and support, facilitating communication among care team members, and connecting patients and families with community resources.

Responsibilities :

• Conduct psychosocial assessments of patients and families to identify needs and concerns.
• Develop individualized care plans in collaboration with the interdisciplinary team.
• Provide counseling and emotional support to patients and families.
• Assist with advance care planning, including discussions about end-of-life preferences and goals of care.
• Advocate for patients and families to ensure their needs are met within the healthcare system.
• Coordinate with community resources and agencies to address practical needs such as housing, transportation, and financial assistance.
• Maintain accurate and timely documentation of assessments, interventions, and progress notes.

• Competitive Salary - Minimum hourly wage of $24.50

Requirements :

• A Bachelor’s Degree in social work, counseling, or related field from an accredited university (Master’s preferred)
• Must have a valid non-restricted driver’s license
• Bilingual in Spanish preferred
• Flexibility and availability to work a weekend schedule if needed.
• At least 1 year of healthcare experience preferred.
• Participation in the annual influenza vaccination program is a requirement for all Sangre employees
