A company is looking for a Care Management Support Coordinator.
Key Responsibilities
Perform outreach to members via phone to assist with care plan next steps and resource connections
Serve as the front-line support for member and provider inquiries, explaining care plan procedures
Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
High School diploma or GED
1 - 2 years of related experience
Knowledge of health plan activities and community resources
Ability to address Social Determinants of Health (SDOH) needs
Familiarity with care plan procedures and protocols
