Direct Hire Opportunity
We are looking for an experienced Infusion Pharmacist to join a home infusion pharmacy in Carmel, IN (a northern suburb of Indianapolis). This role focuses on providing safe and effective pharmacy services to patients in the alternate care setting, while ensuring pliance with all applicable standards and regulations.
Schedule : Monday Friday, 8 : 00 AM 5 : 00 PM
Start Date : ASAP
Key Responsibilities :
Review and assess appropriateness of prescribed therapy and patient suitability for home care.
Develop and maintain patient-specific plans of care, including pharmacokinetic dosing and monitoring.
Perform accurate order entry, dispensing, and pump programming / troubleshooting.
Ensure pliance with USP
standards, clean room regulations, and QA / QC documentation.
Provide patient and caregiver education on medication use, infusion methods, and therapy goals.
Collaborate with physicians, nurses, and other healthcare professionals to optimize outes.
Document therapy progress, munication, and interventions in the medical record.
Supervise and support pharmacy technicians, interns, and students.
Participate in on-call rotation and patient care conferences as needed.
Requirements :
Bachelor of Science in Pharmacy (PharmD or Masters degree in Pharmacy preferred).
Active and unrestricted Indiana Pharmacist license.
Minimum 2 years of recent acute-care experience or at least 1 year of home infusion experience required.
Strong knowledge of intravenous admixture services and IV therapy required.
Ability to evaluate and monitor drug therapy in collaboration with the healthcare team.
Strong problem-solving, multitasking, and supervisory skills.
Excellent verbal and written munication abilities.
