Certified Pharmacy Technician (CPhT) certification and 1+ years Pharmacy Technician experience required. Applicants who do not meet these qualifications will not be considered.

A healthcare organization is seeking an experienced Clinical Pharmacist with expertise in infusion and home infusion pharmacy to support its expanding operations. This role focuses on ensuring the safe and effective management of medication therapies across multiple infusion sites and growing pharmacy locations.

Location : San Rafael, CA

Job Type : Direct Hire (contract-to-hire considered)

Start Date : Immediate availability preferred

Responsibilities :

• Review and interpret prescriptions for accuracy and safety.
• Evaluate patient records to ensure appropriate medication therapy.
• Provide counseling to patients regarding their medications.
• Detect and manage therapeutic inpatibilities.
• Dispense medications to various infusion sites and specialty centers.
• Oversee and verify pharmacy technician activities.
• Collaborate with internal departments including Quality Management, Nursing, Clinical Services, and Reimbursement.
• Manage medication regimens to optimize the treatment and prevention of specialty disease states.
