THE POSITION : INTAKE COORDINATOR
CLASSIFICATION / STATUS : NON-EXEMPT- LEVEL / FULL TIME
IMMEDIATE SUPERVISOR : DIRECTOR OF ADMISSIONS
GENERAL DESCRIPTION OF ASSIGNMENT :
Provides clerical duties involved in the day-to-day facility processes of admissions

ESSENTIAL DUTIES :

New patient intake. Responsible for coordinating the patient intake process, including communication with discharge planners, social workers, physicians, and the patient.
Obtain approval for acceptance from the CFKC clinical and CFKC administrative departments. Negotiate hospital patient support contracts when necessary.
- Maintains open executive communications and working relationships with medical staff, unit personnel, hospital discharge planners, patients, their families, and the public.
- Collects missing pre-admission information from hospital discharge planners and reviews it with clinic nurse manager
- Obtains prospective new patients' insurance coverage information; faxes or scans and emails insurance information to corporate billers for verification
- Confirm a new patient's schedule and appointment time with the nurse manager or charge nurse.
- Promptly notifies nurse manager about insurance verification from corporate billers.
- When patients are admitted, inform hospital personnel of the date of the first outpatient dialysis. Also, provide the time a patient should arrive for consent review prior to the first treatment and the need for patients to have transportation.
- Photocopies and collates patient admission / intake packets
- Faxes and follows-up for secondary insurance coverage requests and updates
- Enters new patient demographic information and ongoing patient changes into the system
- Answers incoming phone calls, as needed; takes and distributes messages.
- Follows HIPAA policy and procedures.
MARGINAL DUTIES :
- Participates actively in staff meetings and assigned committees.
- Cover the duties of the Medical Records Secretary as needed.
- Complete errands as needed.
- Responsible for cleanliness, orderliness, and organization of assigned work area.
- Maintains safe work environment.
- Other duties as assigned.
SUPERVISORY RESPONSIBILTIES : None
TRAVEL : Minimal travel is expected in this position.
EDUCATION and / or EXPERIENCE :
- High School diploma, required
- Associates Degree, preferred
- Minimum of two years' relevant administrative experience
- Demonstrated MS Office proficiencies
- Possession of the following attributes, representable of the office of the Executive Director :

- Clerical : Knowledge of administrative and clerical procedures and systems such as word processing, managing files and records, stenography and transcription, designing forms, and other office procedures and terminology.
