Summary Resilient Healthcare combines virtual and in-person care, utilizing specialized technology to better enhance and streamline the experience for both the patient and the clinician.

Reporting to the Medical Director, the Wound Care Certified Advanced Practice Provider will plan, direct, and evaluate health care need in the home setting independently and / or in conjunction with a referring physician.

Essential Job Functions The ideal candidate will have excellent interpersonal skills to assist with physician and interdisciplinary team member communications.

This person is responsible for providing in-home care to patients across the spectrum of acuity. The candidate will also embrace a new healthcare delivery system that will work to improve the overall availability of healthcare services.

Meets with new patients in their home to orient them about services, to identify needs, and coordinate care with referring physician.

Assessment, thorough documentation in EMR, sharp debridement, I&Ds, punch and shave biopsies as indicated. Provides wound care, consults with ordering physician, receives and writes orders of associated services to improve wound healing outcomes, including radiologic tests, laboratory testing, vascular testing, specialty referrals, and orthotics.

Interpretation of diagnostic tests and recommendation of appropriate follow up. Recommendations for prescription of required medications, equipment and supplies (DME) to facilitate wound healing and prevention.

Conducts support groups in subjects related to physical and emotional health ( i .e., challenge of change, dealing with loss, dealing with chronic illness as caregiver, etc.

Coordinates health promotional activities (providing printed information, routine testing, flu shots, mammograms, etc.

Acts as a resource person for decisions, planning, and policies affecting the health of patients. Maintains patient records.

Records statistics and conducts research as deemed necessary by the Company. Compiles reports concerning care at patient’s home.

Collaborates with staff to identify patients' needs and to help develop plan to meet these needs. Follows patients in hospital at home as needed for continuity of care.

Serves as a member of committees dealing with health care. Able to navigate technology for telehealth visit. Other duties as assigned.

Minimum Qualifications 1. Master’s Degree in Science of Nursing or Physician Assistant Studies or related required 2. Minimum 3 years of advanced practice provider experience required 3.

Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas required 4. Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants required 5.

American Red Cross or American Heart Association Basic Life Support Course (BLS or BLCLS) and Certification 6. American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification 7.

Satisfactory references from employers and / or professional peers 8. Satisfactory employment verification 9. Satisfactory licensing verification 10.

Satisfactory education verification 11. Satisfactory criminal background check 12. Satisfactory drug screen Working Conditions This job operates in a patient’s home medical office setting.

An individual in this position may be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (i.

e. needle sticks, etc.) and chemical hazards. Travel by automobile required. Equipment Used This role routinely uses standard office equipment such as computer, keyboard, telephone, fax machine, copy machine, calculator.

Additionally, this role routinely uses medical equipment such as blood pressure cuff, thermometer, stethoscope, etc. Physical Requirements While performing the duties of this job, the employee is regularly required to stand, walk, reach with hands and arms, twist, stoop, and kneel.

Regularly required to use hands and fingers to handle or feel, and to talk or hear. Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus.
