Clinical Pharmacist - Transplant
Work Schedule :
100% FTE, Day / Evening, 7 : 00am 3 : 30pm / 8 : 00am 4 : 30pm, with rotating evening, 1 : 30pm 10 : 00pm, and every 4th weekend.
Hours may vary based on department needs. You will work at University Hospital in Madison, WI.

We are seeking a Clinical Pharmacist - Transplant to :
- Provide medication therapy management services as part of the interdisciplinary transplant team to patients on inpatient and ambulatory services to assure that drug therapy is safe, effective, efficient, and cost-effective.
- Perform both clinical and distributive tasks necessary to ensure that patients’ medication therapy is safe, effective, timely, and efficient.
- Provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences.
- Provide patient care to a diverse population including kidney, liver, pancreas, heart and lung transplant medical and surgical patients.

Education :
Minimum Bachelor’s degree in Pharmacy or a PharmD degree
Preferred Completion of an ASHP Accredited PGY2 Transplant Residency training program
Work Experience :
Minimum One year of clinical patient care and orders management experience
Preferred Three years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited PGY2 Transplant Residency training program
Licenses & Certifications :
Minimum Licensed to practice pharmacy in the State of Wisconsin or must obtain within 90 days of hire date
Preferred Certification through the appropriate certifying program (Board of Pharmacy Specialties).
