Position : Care Manager MSW F / T Day

Job Type : Full-Time (FTE)

Location : Torrance-CA

Base Salary : $76,000 to $118,000 +Best-in-class benefits

Job Description :

  • Care Manager MSW at our Little Company of Mary Hospital in Torrance, CA. Positions available are full-time and will work 8-hour Day shifts.

Required Qualifications :

  • Master's Degree Social Work.
  • 1 year 1-3 years recent experience in acute care inpatient setting with patient population in area of service.
  • 6 months Advanced communication skills to communicate effectively with patients, hospital staff, physicians and the community.
  • Computer experience / literacy as needed for the job.

    • Preferred Qualifications :

  • 6 months Knowledgeable of community resources with care coordination and discharge planning experience preferred.

    • Candidate Details :

  • Seniority Level - Associate
  • Minimum Education - High School Diploma or Equivalent
  • Willingness to Travel - Occasionally

  • Do you have 2 years of recent experience working directly in Hospital Discharge Planning and Care Coordination
