THE OPPORTUNITY
Bronx Community Solutions seeks a Case Manager to provide extensive case monitoring to program participants and service providers to help promote and facilitate program treatment outcomes of meaningful engagement.
Reporting to the Treatment Alternatives Program Manager, the Case Manager will coordinate the initial assessment, treatment referral process, and compliance monitoring to ensure timely and responsive participant engagement and outcomes.
This position is also responsible for providing support to resource coordinators with court coverage in arraignments, criminal court and supreme court parts as needed.
Fluency in Spanish is strongly preferred.
Responsibilities include but are not limited to :
- Screen eligible misdemeanor, non-violent felony, and violent felony defendants to determine program eligibility;
- Collaborate with treatment service providers to provide overall intensive case management services to help facilitate program referrals and assessments as needed, and follow up with service providers on participants’ progress;
- Monitor and report on meaningful engagement in treatment and work to bring non-engaged program participants into treatment compliance, as needed;
- Provide court reports and recommendations for participants;
- Provide court coverage as needed;
- Maintain appropriate client information in electronic databases and hard files, including the program screening tool, court assessment, screening, and outcome information;
- Assist with the DWI (Driving while Intoxicated) initiative with tracking and documenting monthly progress reports;
- Notify providers of next court dates, outcomes, and treatment intake appointments;
- Participate in on-going trainings and attend staff meetings;
- Additional tasks as necessary.
Qualifications : The ideal candidate will have a Bachelor’s degree or a minimum of 3-4 years of experience working in a social service setting.
Experience working in a court or criminal justice setting strongly preferred. Excellent written skills are a must. Candidate must be skilled communicator able to work in multi-disciplinary setting and maintain strong relationships with multiple agencies and organizations both onsite and in the community.
Bilingual is (English-Spanish) preferred.

Compensation : The compensation range for this position is $52,000 - $63,500 and is commensurate with experience.
