Position Summary : The Clinical Psychiatric Nurse Practitioner will provide high quality care to COH patients including psychiatric evaluation, medication prescription, ordering of labs and tests, admission to hospitals, and development of treatment plans.

This role will promote collaboration on treatment plans with patients, medical and emotional wellness team members, and other family members and / or agencies involved in patients' care.

This position may also be responsible for providing clinical supervision for Licensed Graduates and Master level interns.

This position reports directly to the Director of Emotional Wellness and will receive clinical consult from a COH contracted or employed Psychiatrist.

This is a full-time position based out of our Conway Health and Resource Center in SW, DC.

Salary : This position starts at $115k and up based on experience. Indeed provides their own estimated salary calculator and is not affiliated with COH's range.

Highlighted Duties and Responsibilities :

  • Conducting initial psychiatric patient evaluations to determine medication needs and DSM V diagnosis
  • Conduct ongoing medication assessments, safety planning, and crisis response for their patients
  • Develop a plan for treatment based on assessment data, and provides therapeutic interventions (not therapy) as necessary including the provision of on-going medication management services.
  • Treat patients by utilizing psycho-therapeutic methods and medications; discussing progress toward goals with patients;
  • providing instructions; and monitoring effect of medications;
  • Prescribing medication within the scope of their license and experience for children, adolescents, and adults.
  • Meeting with patients regularly in order to monitor their progress and to make any necessary changes to their medication regimen
  • Documenting evaluations of and interactions with patients, including medication and treatment administered; utilizes the collaborative documentation approach to support patients
  • Completing clinical documentation to assist patients in acquiring and maintaining disability
  • Provide consultation to medical and behavioral health staff as needed internally. Collaborate with external providers, social workers, and or care coordinators as necessary.
  • Participate in weekly supervision and monthly staff meetings
  • Provide monthly in-service training's to staff on various diagnostic and psychopharmacology topics

Requirements

Minimum Qualifications :

  • Master's degree in Psychiatric or Mental Health Nursing from a state-approved and accredited school for professional nursing training and at least three (3) years related experience and / or training.
  • DC License AND DEA / CDS required.
  • Experience providing services to an adult and / or child population with chronic mental illness preferred.
  • Ability to build rapport with patients.
  • Bilingual in Spanish and / or Amharic preferred.
  • Commitment to working with under-resourced populations in an urban setting.
  • Exceptional interpersonal an customer service skills with a proven capacity of working both independently and collaboratively in a fast-paced and rapidly changing environment.
  • Excellent written and verbal communication skills.
  • Ability to travel and work between sites and via telehealth required.

About Us :

Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For over 40 years, we have provided healthcare, housing, and supportive services for under-resourced, underserved and people experiencing homelessness in Washington, DC.

As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC.

Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC.

In 2020, Community of Hope provided about 28,400 medical visits, 7,000 dental visits, and 10,000 behavioral health visits for about 11,000 patients.
