Fourth Street Clinic , in downtown Salt Lake City , has been providing essential health care services to those experiencing homelessness for nearly 35 years.

The Medical Scribe works as part of our medical team to assist providers in the completion of necessary documentation of all patient care interactions, including recording patient history, physical exam findings communicated by patient and provider, and procedures performed by a provider or clinical staff, ensuring that all diagnoses, assessments, and treatment plans are notated within the EMR.
This role is critical for maximizing the utilization of our skilled providers both in-clinic and in mobile outreach.
Role and Responsibilities :
- Perform routine scribe duties.
- Chart review before visits, identifying preventative care interventions.
- Document history and physical exam findings communicated by patient and provider.
- Document procedures performed by a provider or clinical staff.
- Document diagnoses, assessments, and treatment plans as dictated by a provider.
- Ensure PHQ2, Cage-aid, substance use screening tool, and other applicable questionnaires or data are accurately recorded in EHR.
- Facilitate medication reconciliation.
- Seeks clarification as needed when documenting. All charts are to be reviewed by a Medical Provider.
- Report all safety concerns with staff and patients to your supervisor immediately.
- Be an active participant in the clinical team including assisting staff when asked for assistance as well as knowing your scope of practice.
Qualifications and Competencies
Required :
- High school graduate or equivalent
- Knowledge of medical terminology, anatomy / physiology
- Excellent oral and written communication skills (keyboard 55 WPM or greater)
- Strong organizational, problem-solving, and flexibility skills
- Current BLS / CPR certification
- General office skills : phone etiquette, computer skills, and filing
- Expected to be prepared to start shift at scheduled time
- Good organizational skills with an emphasis on accuracy, prioritization, and reliability
Preferred :
- Bilingual in Spanish and English
- Graduate of a scribe training program
- Experience with Epic EHR
- Coursework in health sciences
- Capable of working quickly and efficiently under pressure. Work well with others to organize and run efficiently medical provider's clinic schedule
- Sensitive to the needs of homeless people, but capable of being innovative and assertive with a diverse population in a variety of situations
- Excellent communication skills with staff, volunteers, patients, and all people and community organizations involving Fourth Street Clinic
- Provides input in the development of clinical procedures and setting clinic goals
- Responsible for specific duties assigned to them by supervising personnel
- Keeps customer service and the mission of the organization in mind when interacting with all clients, coworkers, and others
