About the Program :
The RN will be a part of our Enhanced Services Facility. This 16-bed facility provides patients with a smooth transition from a hospital setting by providing assisted living and care for those with complex behavioral challenges.
The main goal of this program is to help individuals participate and stabilize in their community.
General Responsibilities : As part of a multidisciplinary team, assess the physical health needs of residents; make appropriate referrals to community physicians;
provide supervision, management, and administration of medication within the scope of practice
Essential Functions : (Tasks, Responsibilities, and Competencies. May not include all duties of this job)
- Assessment and analysis. Initiates data collection and analysis that includes pertinent objective and subjective data regarding the health status of the residents.
- Uses client data and nursing scientific principles to develop nursing diagnosis and to identify client problems in order to deliver effective nursing care.
- Plan nursing care which will assist residents with maintaining or restoring health and wellness or supporting a dignified death.
- Implements the plan of care by initiating nursing interventions* through giving direct care and supervising other members of the care team.
- Evaluates the responses of individuals to nursing interventions and is responsible for the analysis and modification of the nursing care plan consistent with intended outcomes.
- Assess and determine resident abilities, strengths, support needs, interests and preferences in relation to all needs
- Regularly observe residents for changes in physical, mental and emotional functioning to determine if needs are being met.
- Coordinate health care between the resident, their chosen health professionals, and the ESF.
- Notify Administrator if significant changes in medical conditions occur which may potentially require additional support services.
- Provide instruction to residents and staff for the use of any and all prescribed medications and medical treatments including administering, storage and side effects.
- Timely notify all involved staff of changes in resident's medical needs, and instruct in the implementation of the changes (e.
g. medications, activity limitation, special diet, etc.).
- Supervise and provide medication administration. Administer injectable medications to assigned residents.
- Collect bio-medical information from residents such as blood pressure, heart rate, weight, etc.
- Provide nutritional input for menus and meal planning to ensure resident needs are met.
- Maintain charts and files for all residential residents on a regular basis including the documentation of all appointments, medications, correspondence, and conversations.
- Document in incident reports and health records all observed or reported illnesses, accidents, and changes in functional abilities as well as the dates of actions taken.
- Update charts and files for residents on a consistent and regular basis including the documentation of all resident data, bio-medical information, medications, etc.
- Identify, establish rapport with, and maintain ongoing professional relationships with medical professionals in the community who can or do provide health care services for our residents.
- Be accessible and available for on-call when on scheduled rotation, to address emergency / urgent health care needs of residents.
- Provide consultation and training to ESF staff regarding resident health related issues and questions.
- Oversee the medical work of the LPNs.
- Other duties as required.
Required Knowledge, Skills and Abilities :
Must have demonstrated knowledge of standard care practices
Education : Bachelor of Science or Masters of Science degree in nursing (BSN / MSN) and two years of experience is preferred.
Experience : Experience in mental health setting, with demonstrated ability to relate effectively with people of varied backgrounds, is preferred.
Licensure / Certification : RN per WAC 246-840
Schedule : Monday - Friday dayshift with weekend or overnight coverage as needed.
Hiring part-time to full-time (Flexible)
Other Considerations :
- DSHS background clearance required.
- Ability to communicate effectively orally and in writing
- Ability to lift approximately 20-30 pounds and physically assist unsteady residents.
- Current valid Washington driver's license, with excellent driving record and $50,000 liability insurance. (Preferred, not required)
- Skill using Microsoft Office products.
- Strong verbal and written communication skills, and demonstrated leadership ability.
Compensation :
- $45 - $50 / hour depending on experience.
- Wages are determined by applicable industry experience, education, and certifications / licensure.
