A Plus Care Licensed Home Care Agency looking for a full time Intake Coordinator . The ideal candidate would be bilingual Spanish and English, have previous home care experience, have a professional demeanor, excellent verbal and written communication skills, ability to work well under pressure and comfortable in a fast paced environment.

• Accept intake of cases from all referral sources and log into office’s referral log.
• Compiles new patient information from referral source assuring accuracy in communicating to PCP and Vendors.
• Interact with clients and client families to ensure quality care and client satisfaction,
• Initiates progress toward agency growth, maintaining relationships with referral sources.
• Provide input and quality improvement suggestions regarding intake process to management.

Proficient in Microsoft Office Suite, HHA Exchange

Salary : $19.00 - $21.00 per hour
