WeCare Centers is hiring a Regional Discharge Coordinator who is responsible for coordinating and managing the resident's transition through the continuum of care from admission to discharge and post-discharge follow-up. This role ensures that all discharge planning services are resident-centered, compliant with federal and state regulations, support quality outcomes, maximize benefits and resources, and promote safety and continuity of care. The Regional Discharge Coordinator collaborates regularly with a wide range of interdisciplinary partners, residents, family, community partners and more.

This role is a hybrid position with the opportunity to work remotely, with travel to our communities throughout Western PA as needed on a regular basis.
