Description

The Case Manager is responsible for providing outreach programs and activities, providing social support and counseling to assigned clients, advocating for clients and following-up with assigned client(s) health management / care plans.

Posted pay range is the starting salary. Pay rate offered is based on experience.

Examples of Duties

• Provides case management to client population.
• Partners with client(s) to identify needs, agree on goals, choose treatment methods, and articulate specific treatment outcomes and objectives.
• Coaches client(s) in effective management of their chronic health conditions and self-care
• Assists clients in accessing health related services including obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services
• Acts as an advocate and liaison between the client / family, provider, health plan and community service agencies. Assists clients' with scheduling appointments and assisting with completion of applications for programs for which they may be eligible.
• Documents all client encounters including contact information for both client and relevant contacts related to client care.
• Builds and maintains positive working relationships with clients, providers and community service agencies; work collaboratively and effectively as part of a team.

Experience and Training

High School Diploma or equivalent

One (1) year of experience in a health and / or social services setting

Preferred : Bachelor's degree in social work, psychology, counseling, public health or related field

Certificates and Licenses Required

Valid Texas Driver's License

Community Health Worker (CHW) Certification within 6 months of hire

Preferred : Bilingual / Bicultural
