Lydia House (LH), LLC from Liliuokalani Trust, is directly overseen by the LT Board of Trustees, acting as the Board of Directors. LH serves youth and young adults (YYA) aged 16 to 26 experiencing high-risk factors and involvement in systems of care. The Center supports YYAs with low barrier access to basic needs services through a youth-centered approach. LH aims to stabilize basic needs and support YYAs who choose to thrive. The center operates based on Hawaiian values and a healing-centered approach, emphasizing strengths and innate values of all YYAs, regardless of past experiences and current environment. The Center's engagement activities aim to meet all basic needs. LH also offers the Thriving Supportive Program (TSP), an intensive case management project that supports re-entry into education and the workforce, including financial and housing support. LH maintains 40 beds across 18 dorms to provide stability for qualified young adults (18+) enrolled in TSP and actively engaged in their thriving goals.

Job Purpose

The Pua Lili?u Wellbeing Practice Model underpins all direct service practices within LT. Rooted in Hawaiian cultural foundations and developed through an organizational process, it is a holistic model representing LTs interpretation of key elements that define and maintain well-being. Pua Lili?u aligns all LT frameworks and manuals guiding our direct service practices.

The Pua Liliu Manager (PLM / Case Manager) supports and navigates youth and young adult services at Lydia House. This role involves planning and executing services and activities, fostering partnerships, and supporting the 'kamalii' (youth) in the Engagement Center at LH. The PLM ensures the successful implementation of the Thriving Plan for young adults in TSP, supporting outreach, low-barrier access, and providing a safe, youth-centered environment. Responsibilities include overseeing program delivery, providing mentorship, integrating culturally-centered interventions aligned with the Pua Liliu framework, and continuously improving services based on youth feedback.

Engagement Center

• Creates a safe physical and emotional environment, ensuring low-barrier access for all eligible youth and young adults.
• Develops and supports services that meet basic needs and promote stability, adopting a youth-centered approach.
• Collaborates with team members and external partners to support best practices tailored to youth needs.
• Fosters an environment where youth feel safe to self-direct activities, fostering stability, belonging, and future orientation.
• Conducts community outreach, environmental analysis, and feedback collection to inform service design.
• Collects and reports participant data, collaborating with other teams to utilize this information for impactful service models.
• Supports all activities within the Engagement Center under the supervision of the Lead PLM and Director.
