Under supervision, you will perform a range of functions,

covering multiple pharmacy locations, units or other settings during a variety

of shifts, including but not limited to, float shifts. Your duties include the

preparation and dispensing of drugs in all inpatient settings; review and

verification of medication orders and patient medical records; optimizing safe

medication therapy; review of patient medication profiles; counseling patients

on appropriate drug use; interacting and / or rounding with other health

professionals to help provide safe and effective use of medications; provision

of advanced clinical services (e.g., pharmacokinetic dosing, therapeutic

interchange, IV / PO conversions, TPN and aminoglycoside monitoring, pharmacist

interventions , anticoagulation ) to neonatal, pediatric and adult patients according to

departmental policy and procedures, with possible focus on pharmacy specialty

(i.e., oncology, pediatrics, geriatrics, ambulatory care); oversee dispensing,

return and waste of controlled substances on assigned

shift / unit.Initiate, innovate and complete clinical, operational and / or

management projects, in collaboration with Pharmacy Management. You will work

in assigned centralized and decentralized Pharmacy areas and patient care

units.

Schedule :

7 nights on followed by 7 nights off

Hourly

Salary Range : $86.32 - $107.31 (Union represented, step based salary

depending on years of experience)

Bargaining

Unit : HX-UPTE

Qualifications

We're seeking a dedicated clinical professional with :

• Pharm D. degree and current CA Pharmacy license
• ASHP-accredited residency (preferred)
• 2 yearsinpatientpharmacy experience (preferred)
• Exceptional communication, interpersonal, organizational and follow-through skills.
• Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations.
• Ability to compound intravenous admixtures and other sterile products.
