We are seeking a caring well-organized Case Manager to facilitate successful move-ins, promote housing stability, encourage recovery and independent living skills, and assist with appropriate transitions as needed.

The Case Manager is responsible for:

Conduct a thorough admission for new residents - review resident rights, responsibilities, and community living expectations
Complete initial and ongoing assessments of residents' needs, including housing stability, mental health, substance use recovery goals - meet regularly with residents
Coordinate continued funding for clients in residential program, obtaining written authorizations (including prior authorizations and continuing authorizations).
Ensure adequate documentation of committee activities for accrediting bodies and regulatory agencies
Build and maintain relationships with community partners, including shelters, treatment providers, healthcare providers, workforce programs, and social service agencies

Qualifications for the Case Manager:

Bachelor's Degree in social services or a closely related field
A minimum of one year of experience as a case manager (or closely related position) in a social service environment
Prior experience working in residential programs
Demonstrated ability to successfully collaborate and work productively as a member of a multidisciplinary team.
Excellent written and oral communication skills and interpersonal skills
Ability to work effectively in a busy environment with multiple distractions. Ability to maintain focus and attention for prolonged periods.
