Social Worker

Salary range : $72,000-$100,000 / year

Job Type : Direct Hire

Responsibilities :

• Assess the psychosocial needs of patients and families
• Collaborate with internal and external teams and partners to develop care plans
• Educate patients and families on resources and act as patient's advocate
• Provide therapeutic psychosocial assessments, counseling, and crisis intervention
• Participate in company events and partner programs as needed
• Engage on national and regional committees as required
• Manage communication with external parties and organization to promote company services and programming
• Assist, as needed, with resolving billing and insurance issues

Qualifications :

• Master's degree in Social Work required
• 3+ years of experience in a healthcare setting required
• Valid Licensed Clinical Social Worker in the State of Indiana preferred , but Valid Licensed Social Worker considered if currently pursing clinical license
• Valid Indiana driver's license and insurance automobile required
• Must be able to occasionally travel to patient homes, schools and outreach clinics

This role is in office until fully trained and then there will be potential to work from home 1 day per week.
