Medical Social Worker - Home Health Care

Responsibilities :
- Develop and implement customized care plans for each client based on their specific needs and preferences
- Conduct in-home assessments to evaluate clients' health conditions and recommend appropriate services
- Coordinate with healthcare professionals, family members, and other caregivers to ensure a comprehensive care approach
- Maintain accurate and up-to-date documentation of client interactions, care plans, and progress
- Provide compassionate and respectful care to enhance the quality of life for clients and promote their independence
Qualifications :
- 1+ years of experience in healthcare Social work
- Strong organizational and time management skills
- Proficiency in electronic medical records systems
- Excellent verbal and written communication abilities
- US Work Authorization
- Must be a Licensed Medical Social Worker
- Willing to travel a 20-mile radius from base office to see patients
