Job SummaryProvides parenting and home management education and facilitates access to health and social services for assigned children and families.
Services are provided according to the agency's mission, philosophy, and department goals.Responsibilities and Duties : * Implement the Healthy Families model and curriculum with fidelity and follow all program and agency policies and procedures.
- Conduct visits in a family's home and maintain a caseload of families.* Attend and participate in required virtual and / or in-person training and program meetings.
- Build and maintain relationships with community agencies and partners to share program information and learn about resources available for families.
- Participate in Continuous Quality Improvement activities.* Administer screenings for parents and children.* Completes weekly recruitment strategies within the program's service area, to reach eligible families.
- Supports the eligibility process for interested families including the collection and analysis of required documentation when necessary* Supports families by completing family assessments, encouraging family goals, and providing community resources and referrals that support family well-being.
- Completes ongoing case notes documentation, required forms, and reporting for families on caseload.* Coordinates and participates in all family engagement activities such as parent meetings, workshops, and events.
- Completes additional assignments, tasks, and reports as directedEducation / Experience : * High School Diploma and experience in infant, early childhood, or related area* Bilingual English / Spanish preferred* Demonstrated attention to detail and follow-through skills* Must be able to display verbal and written communication skills that meet the requirements of the position* Highly proficient with Microsoft Office Suite and comfortable with technology in performing job duties* Must possess a valid Level One Fingerprint Clearance Card or be able to obtain one before employment* Must be able to pass a pre-employment drug screen.
