Position Summary

True Assist Home Care is seeking a highly organized and communicative Home Care Intake Coordinator to serve as the first point of contact for prospective clients. In this vital role, you will manage all incoming inquiries, conduct thorough intake assessments, collaborate with referral partners, and guide clients and their families through the initial phases of the home care journey. You will play a key role in ensuring a seamless and supportive transition from initial contact to the commencement of services, keeping families well-informed throughout the process. By fostering trust, empathy, and professionalism, you will contribute to a positive care experience that embodies the core values of True Assist Home Care. Key Responsibilities

  • Promptly respond to incoming client inquiries through phone, email, or referral channels.
  • Effectively communicate service offerings, pricing structures, and care options to potential clients.
  • Conduct timely follow-ups with prospective clients who have not yet initiated care.
  • Coordinate with the care manager or administrator to schedule and perform on-site intake assessments.
  • Ensure accurate and complete documentation within the client intake management system.
  • Partner with scheduling and care management team to facilitate smooth client onboarding.
  • Monitor, track, and report on inquiry volume and conversion metrics.

Qualifications

  • High school diploma or equivalent required; associate or bachelor’s degree preferred.
  • At least one year of experience managing intake procedures in a home care or healthcare environment.
  • Demonstrated ability to interact effectively with customers and provide exceptional service.
  • Strong organizational skills with the ability to multitask efficiently.
  • Excellent communication skills, both written and verbal.
  • Bilingual proficiency in English and Spanish is preferred.
  • Skilled in Microsoft Office with advanced data entry expertise.
  • Compassionate, professional, and client-focused demeanor.
  • Preferred Skills

  • Solid understanding of operational workflows and compliance requirements related to non-medical home care services.
  • Working knowledge of commercial insurance, private pay models, and Medicare, including relevant regulations affecting insurance verification within the agency.
  • Demonstrated experience in managing and coordinating client intake processes.
  • Exceptional organizational and analytical skills, combined with strong communication and customer service capabilities.
  • Excellent interpersonal skills with a proactive mindset and the ability to work independently with minimal supervision.
