JOB SUMMARY

This position ensures patients have been cleared for specialty services office visits. Point person for all hospitals, department and other health care facilities pertaining to questions / concerns on all provider orders / referrals.

Provides support to clinical staff in order to facilitate the administrative components of clinical referrals for various services.

Acts as a liaison between hospital, physicians, health plans, vendors and patients, or other referral services. . Manages all outgoing referrals to appropriate agencies to assist the client in achieving the goals and objectives identified in their Care Plan.

Qualifications

Minimum Requirements

Education

Associate’s degree or equivalent knowledge in ambulatory environment.

Experience

Five years experience working with referral processes.

Familiarity with EPC EHR system

Language Skills

Strong verbal and written English and Spanish communication skill.

Preferred Requirements

Experience

Experience in ambulatory healthcare environment with front and back end office process involving patient scheduling, registration, information flow, billing / coding and office systems.

Licensure, Certification, Registration

Completing of training in EPIC and the launching of electronic health records and EPIC reporting across a healthcare system.

  • Strong Microsoft office and multi-media curriculum development systems ability.
  • Ability to work effectively with system vendors, IG consultants, team members and any given HHC affiliate.
