State Licensed Transitions of Care Associate

A company is looking for a Transitions of Care Associate to support members transitioning from hospital care.

Key Responsibilities

Check in with patients post-discharge to ensure they have the necessary support

Coordinate services and care for members with affiliated vendors

Assist in scheduling post-discharge appointments with healthcare providers

Required Qualifications

Minimum of 1 year of experience in a Transitions of Care program

Medical Assistant Certification

2-3 years of experience in healthcare, managed care, home health, or community health settings

Fully bilingual in English and Spanish (desired)
