Responsibilities
Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology.
Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.
- Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.
- Recommend formulary alternatives when appropriate.
- Compound and dispense medications for accurate delivery to patients. Provide drug information and education to medical staff and patients.
- Communicate with physicians and other clinicians as necessary to facilitate care.
- Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
- Assist with academic functions such as serving as a preceptor for pharmacy students. May float to provide coverage of pharmacy needs.
Provide patient counseling when required.
Requirements
Licensure / Certification / Registration :
Pharmacist credentialed from the Indiana Board Of Pharmacy obtained prior to hire date or job transfer date required.
Education :
Bachelor's or advanced degree in Pharmacy required.
Additional Preferences
This position is for an Ambulatory Care Pharmacist Specialist at a primary care center focused on teaching and providing care to vulnerable populations as well as various Ascension Medical Group Sites, providing collaborative drug therapy management, working closely with internal medicine and family medicine residents, and precept pharmacy residents, all as part of the ambulatory care team of pharmacists.
We are seeking a candidate that has completed PGY2 residency in Ambulatory Care or equivalent clinical experience in Ambulatory Care.
Additional Responsibilities :
- Utilize pharmacist collaborative practice agreement with Providers
- Assist with advanced prior authorization where clinical expertise required
- Provide patient education
- Provide disease state visits with patients
- Provide education to interdisciplinary team members as well as precept of APPE students, PGY1 and PGY2 pharmacy residents
- Facilitate and participate in quality and safety improvement
- Identify areas for fiscal stewardship where able
