Community Medical Services (CMS) is hiring a Clinical Documentation Coordinator. Under the supervision of the Clinic Manager the Clinical Documentation Coordinator is responsible for managing the end-to-end flow of client documentation to ensure all required forms are completed signed uploaded and accurately linked to the correct client chart within established timeframes. This role works closely with Counselors Client Service Coordinators and Nursing staff on a daily basis to maintain timely and compliant records that support quality client care.

Responsibilities :
Performs general office duties such as document preparation scanning faxingEnsures confidentiality of all client and employee information.Collect required forms (in-person and remote).Track missing items and rout to the right person (client / counselor / nurse) for completion.Collect signatures on required documents.Monitor signature status and escalate delays.Scan documents and link them to the correct client chart.Facilitate internal transfers to other CMS locations as well as transfers to external MAT programs.Maintain and track forms for transfer process.Assist with completion of required intake documents and forms collecting signatures and routing as appropriate.Assist counselors with scanning / uploading / faxing forms for courtesy dosing and request exceptions for take-home medication policy.Assist Client Service Coordinators with the patient flow of a high volume of clients in a fast-paced environment.Maintains a welcoming lobby and reception area by ensuring areas remain clean and organized.Recovery Connect engagement app and help them navigate it as needed.Requirements
High school diploma or GED requiredBilingual in English / Spanish preferred1 years of experience in customer service required1 years of front desk experience requiredFront desk experience in a Hospital or multi-physician practice preferredBehavioral health work experience preferredSkills :
