Our client is looking for Registered Nurses to join their growing home health care team in the Fort Collins, CO area.

The Role :

This role is 70% in person, 30% remote

Our client is looking for a reliable and passionate registered nurse to join their home health care team in the Fort Collins, CO area as an RN Case Manager.

You will help develop a plan of care and deliver high-quality, skilled nursing for the families of children with intellectual and developmental disabilities (IDD).

In this Role you will :

• Visit the homes of patients every 60 days
• Create care plans, provide care in-home to children, and oversee patient progress
• Inform parents about their children’s health and provide coaching support
• Leverage telehealth to scale the impact of your time, communicate with the care team, and interact with families
• Be the critical voice for working to help children meet great health outcomes and furthering our client's mission

Requirements :

• Must be a registered nurse with at least 2 years experience and currently licensed in the state of Colorado or hold a compact license
• Experience working with and training family CNAs, preferred
• Ready to work in a fast-paced, start-up environment
• Must live in the Fort Collins, CO area or be willing to relocate
• Must have CPR certification
• Home care experience REQUIRED
• Pediatric experience, preferred
• BLS certified, required
• PALS certified, preferred

• Daily Schedule :

Monday :

• Mandatory Nurse meeting in office in Denver 9am-10 : 30am
• The rest of the day is admin day for nurses to work for home and get all their tasks completed

Tuesday-Friday :

• In-person recertifications - Max 3 per day. Usually 2 recerts per day
• In-person start of care assessment - Max 3 per week. Usually only 1 per day
• The rest of the time is working remotely to chart, connect with families, and get case management work completed
