Job description
JOB SUMMARYWorks under general supervision to provide Diabetes Self-Management Education and Support (DSMES) for patients within a designated department or specialty area.ESSENTIAL FUNCTIONSJob Duties
Conducts patient assessments, prioritizes care needs, and involves family members in the education process in an outpatient setting. Ensures effective communication with the care team.
Develops and implements a comprehensive educational plan that caters to the specific needs and limitations of patients, based on orders from Physicians or Qualified Non-Physician Practitioners.
Ensures all documentation adheres to organizational guidelines and ADA and ADCES standards. Utilizes time management skills to complete tasks efficiently.
Provides education in various settings, including one-on-one sessions and group classes. Facilitates group discussions as required.
Contributes to the development and maintenance of program accreditation plans in accordance with ADA and ADCES guidelines. Ensures adherence to program plans for maintaining excellence and consistency.
Initiates contact with the care team as needed and involves the unit supervisor or manager for issues that require senior-level intervention.
Actively contributes to the program's quality and performance improvement activities.
Continually enhances knowledge and clinical skills through Continuing Education Unit (CEU) programs, personal study, and staying updated with clinical standards. Maintains practice standards consistent with ADA and ADCES recommended competencies.
Develops educational materials in line with ADA and ADCES guidelines and collaborates with the supervisor / manager and team members for feedback on new materials.
Participates in health system initiatives through formal appointments or as a representative of the unit. Engages in community screenings and health-fair initiatives as appropriate.
Demonstrates adaptability in managing daily assignments and scheduling, accommodating cancellations and urgent appointments as they arise.QUALIFICATIONSEducation and ExperienceRequired
EducationBachelor's Degree in related field. Graduate of an approved / accredited nursing program.
MSN preferred.
ExperienceSpecialty areas may have increased experience requirements as dictated by clinic or standard of care.
Diabetic Educator experience strongly preferred.
Licenses and Certifications(RN) REGISTERED NURSE current State of Texas Nursing license Upon Hire(BLS) BASIC LIFE SUPPORT accredited by the American Heart Association (AHA) or American Red Cross (ARC Upon HireMust hold and maintain Certified Diabetes Care and Education Specialist (CDCES) or(BC-ADM) Adv Diabetes Mgmt for CNS & NP credentialed within 1 YearPreferred
Experience1 year clinical nursing experienceKnowledge, Skills and Abilities
