This is for our client’s 501 (c)(3) nonprofit organization that provides housing and mental health support to individuals with intellectual and developmental disabilities.
This job is HYBRID, meaning predominantly working on laptop and cell phone servicing individuals in Sonoma and Solano counties. Some office visits may be needed for occasional team meetings. Best if live in or close to those counties,

Job Duties & Responsibilities :
- Responsible for the entire case management process, including assessment and evaluation, planning, authorizing, advocacy, monitoring, reviewing documentation for completion.
- Responsible for providing a safe and welcoming environment for individuals , developing case plans according to the needs of each individual and providing ongoing support as they work toward achieving goals.
- Conduct initial and ongoing assessments and follow-up visits to each individual.
- Assess individuals’ needs and link them with outside services such as primary medical care, outpatient treatment, government financial assistance etc.
- Maintain all documentation and contacts as required by law, regulation and policy
- Advocate on behalf of the organization’s individuals to protect their rights under the law and ensure that they receive those services and benefits to which they are entitled.
- Collaborate with other departments and outside providers to support and advocate for the organization’s individuals.
- Respond to crises, including providing de-escalation intervention and support to highly agitated individuals.
- Keep supervisor informed of the status of the caseload and unmet needs in the community.
- Ability to cover evening and weekend shifts with certain caseloads.
