Full Circle is looking for a Model of Care manager to support the development of core service delivery models for enhanced care management, community health worker and community supports. Our ideal applicant is a licensed clinician or nurse. The position will report to the Director of Operations. This an exciting opportunity to be at the nexus of California’s CalAIM Medi-Cal reforms to help make the systems work better for at-risk or justice-involved youth. This position will be responsible for helping Full Circle reach our goal of having more than 5,000 youth served through various CalAIM service lines by 2025.
Responsibilities :
- In collaboration with key internal and external stakeholders, support the ongoing development of Full Circle’s CalAIM service models of care that reflect the lived experience and honors the knowledge of participating CBOs, including for specialized populations like individuals involved with child welfare and juvenile justice systems. This includes setting operational, administrative and programmatic key performance indicators and implementing quality improvement programs based on continuous monitoring.
- Build the training curriculum and develop trainings for a wide range of providers including : First Fives, Specialty Mental Health providers, FFAs, and others.
- Represent Full Circle and providers at key meetings to inform and shape ongoing policy development in this space, such as Department of Healthcare Services (DHCS), County, Path Collaborative and other key meetings.
- Oversee the program certification process for new providers.
- Support the development of outcome metrics and monitoring processes to track program success and identify opportunities for improvement.
- Work closely with the quality department to ensure providers are implementing strategies that result in accurate data collection and continuous quality improvement toward expected outcomes.
- Cultivate and maintain positive relationships with County partners to meet the needs of the system involved populations to include, coordinating with internal and external departments to ensure information is needed to implement the appropriate interventions to ameliorate systemic issues and barriers, identify overlap, identify potential conflict, and assist with resolutions
- Host monthly learning communities with providers that are specific to developing program models and driving improved outcomes for various populations of focus, including justice involved, child welfare involved, families and / or youth experiencing services, and between like providers, such as family resource centers, foster family agencies, and domestic violence programs.
- Be a subject matter expert for all CalAIM services, Enhanced Care Management, Community Health Workers, and Community Supports, with providers, managed care plans and policy makers.
- Support the development of training and communication materials and participate in forums to share core principles and best practices for CalAIM service adoption, based on the provider experience.
