Job description
A company is looking for a Platform Coordinator in Home Health Care.
Key Responsibilities
Support Care Coordinators with platform referrals and process patient referrals from various systems
Communicate with team members regarding patient updates and respond to inquiries about referral status
Ensure compliance with HIPAA regulations and meet productivity goals set by the supervisor
Required Qualifications
High School Diploma (or GED); Associate's or Bachelor's Degree in a related field preferred
Ability to manage multiple tasks and work under pressure effectively
Experience with EMR databases such as Allscripts, Navi, Epic NCH, and PowerChart
Strong organizational and time management skills
Proficient in Microsoft Word, Excel, and Outlook
