Liv Well is an outpatient treatment center in the East Valley that focuses on providing growth-oriented and interactive group counseling and skill building services for children in an environment that promotes individualism and provides children with the tools they need to live healthy lives.A Case Manager ensures clients receive comprehensive and individualized care. Key responsibilities include conducting intake processes, administering CALOCUS assessments to determine appropriate levels of care, and developing treatment and discharge plans that address the unique needs of children and families. The Case Manager coordinates supportive services, facilitates communication with families and caregivers, and ensures goals are progressing through regular assessments and plan updates. Primary Responsibilities : Intake, Assessment, and Treatment and Discharge Planning : Administer the CALOCUS assessment to determine the appropriate level of care for clients, considering clinical, functional, and environmental factors to guide service allocationComplete initial intake and paperwork for clients admitted to programming servicesConduct comprehensive assessments to identify the mental health needs of children and develop individualized service plansUpdate CALOCOS, safety plans, risk assessments, treatment plans and clinical assessments every six months, or as needed, to ensure goals are progressing or being adjusted for appropriateness of servicesEnsure the development and implementation of transition, discharge, and aftercare plans prior to discontinuation of behavioral health servicesService Coordination : Coordinate supportive services clients / families may need.Communicate with guardians / caregivers weekly on the progress and effectiveness of treatmentMeet with every assigned family on caseload at minimum once monthly for check-in.Participate in departmental client staffing meetings and debrief with the BHT team to understand client progress towards goalsClient Advocacy : Advocate for clients needs and ensure they receive appropriate services and support.Assist families in accomplishing tasks that require guidance or extra help (e.g., AHCCCS appointments, crisis intervention)Facilitation : Facilitate CFT meetings to ensure coordinated careAttend quarterly school staff meetings and provide updates on client progressCommunity Outreach : Develop and maintain positive relationships with community resources and referral sourcesDocumentation : Maintain accurate and up-to-date client records, including case notes, service plans, and monthly progress reports in accordance with state regulations and company policyCollaboration : Work closely with families, guardians / caregivers, and other professionals to ensure a holistic approach to careCrisis Intervention : Provide crisis intervention and support to clients and their families as neededRequirements : Spanish speaking ability highly preferredAbility to prioritize and delegate tasks Strong planning and organizational skills Ability to learn industry rules and guidelines Acute observation skills.Planning and task management skills.Works well independently and with a team.Ability to maintain composure under stressful conditions.Ability to relate to guardians / families of diverse backgrounds and assess strengths and opportunities for growthStrong verbal and written communication skills with clients, families, and professionals.Critical thinking, problem-solving, and computer skills (Google Workspace, Adobe PDF)Great team communication and interpersonal skills Education & ExperienceBachelor's degree in social work, psychology, or a related field is preferred.
