PRN - Part-time Home Health OT
Top Home Health Care Agency seeks experienced OT to provide occupational therapy in patient's place of residence and facilitate coordination of care with RN, home care team and / or Physician. on a part-time basis. Must be graduate of accredited occupational therapy (OT) program. Current license to practice as OT in Indiana. Minimum of one year clinical experience (prefer past experience in home care). Excellent documentation skills required for compliance and proficient with computer charting systems.
Job Description
1. Performs occupational therapy evaluations, diagnostic tests, skilled procedures and ongoing evaluations under a medically approved plan of care.
a. Completes assessments timely.
b. Develops, reviews and revises therapy care plan as appropriate to patient's needs and physician orders.
c. Advises, consults and teaches treatment procedures and supportive activities to patients, family / caregiver and agency staff.
2. Coordinates patient care in a timely manner.
3. Promotes personal safety and a safe environment for patients and coworkers. 4. Implements appropriate standard precautions and other infection control practices.
5. Completes and submits required documentation in an accurate, complete and timely manner.
6. Promotes effective communication, both verbal and written.
7. Maintain expected standards of quality care within the scope of OT practice.
8. Self evaluates the effectiveness and efficiency of own professional practice in relation to home care.
9. Participates in interdisciplinary evaluation of the patient care and the development and / or changes to the plan of care.
10. Participates in QAPI process.
