Job descriptionVerify and update patient histories, medical records, and relevant information. Use EMR / CRM systems to document patient interactions and maintain accurate records. Identify and remove records of deceased patients or those who have moved out of coverage areas and document accordingly. Coordinate with other clinics to assist with appointment scheduling and logistics. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily Adaptability when dealing with constantly changing processes, computer systems Critical thinking skill. Proficient experience utilizing EMR, patient charts, Microsoft Office Suite with emphasis on Excel and Outlook Effectively communicate both orally and written, to a variety of individuals Ability to multitask and ability to make calls in a timely manner

Job Description : Outreach Calls :
- Contact patients who are overdue for appointments (last seen 6 months to a year ago).
- Engage in friendly conversations, encourage and set appointments, and address any concerns.
- Transition Support :
- Reach out to patients whose physician has left the practice.
- Understand their new plan of care and guide them to physicians similar to their previous provider. Set appointment to see new provider.
