The Social Services Designee plays a vital role in supporting residents and families throughout their stay. This position assists with psychosocial assessments, discharge planning, adjustment support, and ensuring residents receive compassionate, coordinated services.
You will collaborate closely with nursing, admissions, and administration to help residents feel safe, respected, and supported.
Conduct psychosocial assessments for new admissions
Provide support, counseling, and advocacy to residents and families
Assist with discharge planning and community resource coordination
Maintain documentation in compliance with state and federal regulations
Participate in care plan meetings with an interdisciplinary team
Address resident concerns and help promote a positive, respectful environment
Support transitions of care and assist with family communications
Must hold an active LCSW or LICSW license (required – applications without this will not be considered)
Minimum of 1 year of experience working in a Skilled Nursing Facility (SNF) (required)
Experience in Social Services or Human Services in a healthcare or long-term care setting
Bachelor’s in Social Work (BSW) or related field
Strong communication, empathy, and problem-solving skills
Ability to work collaboratively in a fast-paced, resident-centered environment
