The Social Worker collaborates with an interdisciplinary team in the delivery of psychosocial wellbeing needs of the resident, family members and significant others.
Promotes a team concept with other departments and disciplines through demonstrating good interpersonal skills.
JOB DESCRIPTION
- Maintains constant interaction with other disciplines in the needs of the residents
- Gather appropriate social information at time of admission
- Participating fully in the care planning process
- In collaboration with interdisciplinary team to ensure continuity of care
- Works with Social Worker Consultant to provide yearly in-service education
- Discharge planning
QUALIFICATIONS
- LSW license required, LICSW preferred
- Bachelor’s degree preferred
- 1+ yrs. experience in LTC
