Job Description
As a Medical Social Worker (MSW) at Ovation, you will play a pivotal role in ensuring the emotional, social, and psychological well-being of our hospice patients and their families.
You will collaborate with an interdisciplinary team to provide holistic care, offer counseling and support, and address psychosocial factors affecting patients' end-of-life experiences.
Responsibilities
- Conduct comprehensive psychosocial assessments of patients and their families to identify their emotional, social, and cultural needs.
- Develop and implement care plans that address the psychosocial, emotional, and spiritual aspects of patient care.
- Provide counseling and emotional support to patients and families facing end-of-life challenges, grief, and loss.
- Collaborate closely with the hospice interdisciplinary team, including physicians, nurses, chaplains, and other professionals, to ensure coordinated and patient-centered care.
- Assist patients and families in accessing community resources, financial assistance, and additional support services as needed.
- Maintain accurate and timely documentation of all patient interactions and care plans.
- Participate in team meetings, case conferences, and in-service education to stay current on hospice regulations and best practices.
- Promote a compassionate and inclusive environment that respects the diverse backgrounds and beliefs of patients and their families.
Qualifications
- Master's degree in Social Work (MSW) from an accredited program.
- Current licensure or eligibility for licensure as a Licensed Clinical Social Worker (LCSW) in Colorado.
- Previous experience in hospice or palliative care is preferred.
- Excellent communication and interpersonal skills.
- Compassion, empathy, and a strong commitment to patient-centered care.
- Ability to work effectively in a multidisciplinary team.
- Strong problem-solving and advocacy skills.
- Proficiency in documentation and record keeping.
