POSITION SUMMARY : Provides a variety of support services to homeless or near-homeless Veterans; assists in coordinating services and communication among landlords and CNCAP; provides financial assistance for eligible persons; identifies and accesses resources; seeks to expand program and resources independently and confidently; promotes and maintains public relations; and completes forms accurately and timely. Provide case management, and assists in recruiting and enrolling Veterans into the Supportive Services for Veterans and their Families Program.
ROLE AND RESPONSIBILITIES
- Responsible for completing the agency intake form and inputting it into Clarity
- Provide ongoing support to veterans and document via home visits, phone calls, and written correspondence with goal attainment of 65% or above
- Maintain a non-judgmental attitude toward clients and other CNCAP staff
- Ensure clients are assessed through the SSVF process to determine possible assistance through resources
- Create family personal plans to develop short and long-term attainable goals with veterans
- Ensure files are complete with the necessary paperwork for veteran families participating in the program
- Adhere to all CNCAP-related policies and procedures
- Promote the program and agency in a positive and professional manner
QUALIFICATIONS
SKILLS & ABILITIES
SCHEDULE : Monday-Thursday : 7 am - 5 pm
REPORTS TO : SSVF Case Manager Supervisor
JOB STATUS : Full-Time, 40 hours per week
PAY : $16.50 - $18.25 (depending on qualifications)
BENEFITS :
FLSA STATUS : Hourly / Non-Exempt
