Role Overview :
As a Family Medicine Physician at Tapestry 360 Health, you will provide comprehensive primary care to patients
of all ages, ensuring their physical and emotional well-being. You will be a critical part of a multidisciplinary
team, delivering care that spans prevention, diagnosis, treatment, and follow-up, with a focus on addressing
health disparities in our community. You will also play a key role in community health initiatives aimed at
improving overall well-being through outreach and education.
Key Responsibilities :
o Comprehensive Care : Provide patient-centered primary care, including exams, diagnostics, treatment,
and follow-up, with a focus on prevention and chronic disease management.
o Team Collaboration : Collaborate with nurse practitioners, physician assistants, and other health
professionals to ensure seamless, coordinated care.
o Leadership & Supervision : Provide clinical supervision to nurses and medical assistants, contributing to
the professional development of the healthcare team.
o Community Engagement : Participate in public health initiatives, addressing issues such as immunization,
diabetes management, and hypertension, while ensuring access to care for all community members.
o Documentation & Compliance : Maintain accurate electronic medical records (EMR) and ensure
compliance with healthcare regulations, including privacy standards.
o Continuous Improvement : Actively engage in quality improvement activities, including peer reviews and
protocol development, to enhance patient outcomes and operational efficiency.

Qualifications
o Education : MD or DO from an accredited medical school, with Board Certification (or eligibility) in Family
Medicine.
o Licensure : Active Illinois medical license.
o Experience : Previous experience in a community health setting or working with underserved
populations is highly desirable.
o Skills : Strong interpersonal, diagnostic, and clinical decision-making skills. Familiarity with EMR systems
and a commitment to high-quality, evidence-based care
Salary and Benefits :
o Compensation : The annual salary range for this position is $160,000.00 to $200,000.00, based on
experience and qualifications.
