Job description

Regional Program Coordinator (Northern California)

APHealth is transforming how care is delivered to California's most vulnerable communities. Through ECM, CS, and CHW services, APHealth helps individuals facing complex medical, behavioral health, housing, and social challenges access the support needed to improve stability, health, and well-being. We believe care should be delivered where people live, not from behind a desk.

The April Parker Foundation (APF) is seeking an experienced, mission-driven Regional Program Coordinator to lead Enhanced Care Management (ECM), Community Supports (CS), and Community Health Worker (CHW) services in the Northern California Region.

This is a field-connected leadership role for professionals who thrive at the intersection of operations, people leadership, and direct service. Each coordinator supervises Care Managers, oversees program performance, ensures compliance and quality standards, and maintains a small caseload of approximately 20 members.

Candidates must reside in the Northern California region they supervise and be comfortable traveling extensively throughout that region, including overnight travel as required.

What You'll Do

Regional Leadership

  • Supervise, coach, mentor, and support Care Managers.
  • Conduct performance reviews and provide ongoing staff development.
  • Monitor productivity, documentation quality, member engagement, and outcomes.
  • Lead team meetings, case reviews, and quality improvement activities.
  • Support recruitment, onboarding, training, and field coverage.

Program Operations

  • Coordinate day-to-day regional operations for ECM, CS, and CHW programs.
  • Ensure compliance with CalAIM, DHCS, HIPAA, managed care plan, contractual, and APHealth requirements.
  • Monitor regional performance metrics, caseloads, outreach activity, documentation, and program outcomes.
  • Implement workflows, standard operating procedures, and continuous improvement initiatives.
  • Support audits, chart reviews, corrective action plans, and funder or partner reviews.

Direct Member Services

  • Maintain a direct caseload of approximately 20 members.
  • Complete outreach, assessments, care plans, care coordination, follow-up services, documentation, and case closure activities.
  • Model best practices in member engagement, service delivery, and documentation.
  • Support service continuity during staffing shortages, transitions, or coverage gaps.

Community Engagement

  • Represent APHealth throughout the assigned region.
  • Build relationships with managed care plans, healthcare providers, community-based organizations, and service partners.
  • Participate in community meetings, outreach events, and collaborative initiatives.
  • Support regional growth and partnership development efforts.

Travel Requirements

  • Reside within the assigned Northern or Southern California region.
  • Travel frequently throughout the assigned territory.
  • Conduct in-person member visits and field-based staff support as needed.
  • Attend community, provider, partner, statewide training, and leadership meetings.
  • Be available for overnight travel throughout California as required.

Qualifications

  • Bachelor's degree in Social Work, Public Health, Human Services, Psychology, Nursing, or a related field
  • At least five years of experience in care management, healthcare, behavioral health, case management, or social services.
  • At least three years of supervisory, lead, or management experience.
  • Knowledge of CalAIM, Enhanced Care Management, and Community Supports programs.
  • Strong leadership, communication, organization, and problem-solving skills.
  • Experience working with vulnerable populations or individuals with complex needs.
  • Proficiency with electronic health records, care management platforms, and performance reporting systems ideal candidate.

Preferred Qualifications

  • Master's degree in a related field.
  • Experience in a community-based organization, FQHC, managed care plan, or health system.
  • Bilingual in English and Spanish.
  • Community Health Worker certification.
  • Experience leading geographically dispersed teams.
  • Experience managing or scaling regional healthcare or social service programs.
