Eastern Healthcare is looking for a Social Worker Assistant to provide direct resident care duties under the supervision of licensed nursing personnel to our residents and patients. In this Social Worker Assistant role, you will assist with promoting a compassionate physical and psychosocial environment for the residents.

Job Description :

Assist the Social Services Director in identifying and providing for each resident's social, emotional and psychological needs, and the continuing development of the resident's full potential during his / her stay at the facility and to assist in the planning for his / her discharge.

Responsibilities of the Social Worker Assistant :

• Assist in collecting social history data and in the development of a comprehensive discharge plan, including the MDS and Care Plan for new admits, as directed by the Social Services Director.
• Orient and assist new residents in adjusting to the facility. Assist residents and families with adjustment to changes, including room changes.
• Assist families with the engraving and / or marking of resident's personal items.
• Document progress notes, which relate to each resident's care plan when necessary and within policy timeframe
