CareSTL Health seeks a full-time Referral Coordinator to perform administrative tasks, make recommendations to people in need of services, as well as relay important information to companies and organizations.
Essential Functions :
- Coordinates internal and external referral authorizations and tracks specialty care referrals to show trends of patient compliance / noncompliance with providers' recommendations for specialty care to ensure timely access to care of our patients to outside facilities
- Provide support to hospitals, public and private insurers so that referral authorization can be obtained in a timely manner to ensure the best health outcome for our patients and their families
- Actively participates in the performance improvement committee and performs as a cohesive team member as well as communicates well with public and private insurers and hospitals
- Provides a monthly report on the status of all referrals on an ongoing basis and actively tracks patient's referral to outside agencies and maintain a referral log for tracking in EMR
- Coordinates multiple agency services for the benefit of the client while assisting patients in problem-solving potential issues related to the health care system, financial or social barriers (e.
g., request interpreters as appropriate, transportation services, or prescription assistance)
Other Duties :
- Establishes linkages among service providers and possesses the ability to function independently
- Reinforces client responsibility for independent compliance with keeping appointments
- Evaluates client progress in accessing appropriate medical care and other needed services
- Maintains and updates technical knowledge and skills
Qualifications :
- Education : High School Diploma or GED. Graduate of an accredited school of Medical Assistant (preferred).
- Experience : 1-2 years of experience in a community health center and / or medical office setting
- Other : Current CPR Certification (required). Valid registration as a Registered Medical Assistant in the State of Missouri (preferred).
Must have reliable transportation.
A Successful Candidate has :
- Knowledge of information and techniques for providing excellent patient care
- Knowledge of administrative and office procedures and systems such as electronic transmission via facsimile and / or internet
- Good computer skills including the use of office procedures and terminology
- Good self-motivational and organizational skills
- Ability to maintain confidentiality
- Ability to work all health center sites
- Understanding of the complexities of integrating services
- Knowledge of data management through the use of IT
Additional Information :
- Clinic hours are Monday - Friday 8 a.m. to 5 p.m.
