Job Description :
Support with CMS Star Improvement program including but not limited to
Responsibilities :
- Outreach to Pharmacy’s to support the Pharmacy teams outreach efforts and close the gaps
- Outreach to members across Medicare business lines to close gaps in care
- Respond to any follow-up calls from members or providers on the outreach effort
- Support members with scheduling PCP appointments and if necessary, other specialist appointments (as requested)
- Support with mail merges for select quality measures
- Manage outreach spreadsheets clearly documenting efforts and reporting to management on a regular basis
- Conduct Star MTM program outreach
- Other Star-related and administrative responsibilities as deemed necessary
Experience :
- 2-4 years of experience preferably in Healthcare or managed care or Business related field .
- Skilled in MS Word, Excel, Powerpoint etc.
Skills :
- Good Communication skills
- Strong attention to detail
- Multitasker and ability to work within a team environment
- Strong experience with MS Excel
Education :
Bachelors degree or equivalent experience
