JOB TITLE : Clinical Nurse Coordinator

EMPLOYER : Twelfth One, LLC dba Aspen Infusion

DEPARTMENT : Nursing

REPORTS TO : Director of Nursing

LOCATION : Chandler, AZ

SALARY : $80,000-$85,000 per year based on experience

  • IN-OFFICE ONLY. NO REMOTE CAPABILITY IN THIS POSITION

SUMMARY : The primary responsibility of a Clinical Coordinator includes processing all day-to-day workflow : Plans of Care, completing medication reconciliation, reviewing MD orders, reviewing, and processing new referrals by evaluating clinical information; including but not limited to H&P, line information, consults, appropriateness for home infusion, etc. completing in-home visits as needed. Successful outcomes of this role include accountability for timely and accurate processing of all daily workflow as identified above, accurate and timely documentation of any in-home visits, compliant practice with excellent survey outcomes, positive problem-solving, excellent staff training / support / productivity, and satisfaction.

DUTIES AND RESPONSIBILITIES :

  • Responsible for functions, activities, and evaluation of all workflow to assure documentation of quality patient care and conformity to agency policies, procedures, and professional standards.
  • Reviews patient assessments, plans of care, and re-certifications with staff to verify, instruct, and assist in promoting effective, quality patient care and clinical documentation.
  • Assists other staff in evaluating or planning for patients.
  • Assists with case conferences as needed and assists the DON in planning and facilitating department meetings.
  • Works to develop and maintain positive and effective communication with physicians and among all Aspen Infusion staff. Work as requested, with hospital discharge planners, nurses, and social workers to plan and organize home services upon patients' discharge. As requested, attends meetings to facilitate community relations.
  • Responsible for reviewing and maintaining agency productivity standards. Provides regular feedback to DON regarding productivity, and Quality of Documentation.
  • Assists with insurance re-authorization process when clinical expertise is required by the insurer for such re-authorizations.
  • May be asked to act as Admin on-call on an "as needed" emergency basis only.
  • Supports the Company's Mission and Value Performance Standards.
  • Performs all other duties as assigned by DON.
  • Effectively formulates, revises, represents, and supports the Mission, policies, and goals of Aspen Infusion in the implementation of the Home Health program.
  • Assists in interviewing and hiring professional staff as requested.
  • Involvement in the Performance Improvement process for the organization.
  • SUPERVISORY RESPONSIBILITIES :

  • This job has no supervisory responsibilities.
  • Requirements

    QUALIFICATIONS :

  • RN required; BSN preferred
  • Current AZ licenses.
  • Knowledge of home infusion regulations and third-party reimbursement.
  • Ability to communicate effectively.
  • Ability to document concisely, accurately, and timely.
  • Excellent organization and training skills.
  • Valid driver's license, reliable auto, and proof of current and adequate auto insurance based on agency requirements.
  • Proof of TB screening.
  • Current BLS Certification
