Assistant Family Living (AFL) Provider Location: Greensboro, NC (and surrounding areas)
Company: CareLink Solutions, Inc.
Employment Type: Full-Time / Independent Contractor (based on placement)

About the Role

CareLink Solutions, Inc. is seeking compassionate, dependable, and dedicated individuals to become Assistant Family Living (AFL) Providers. This unique and rewarding opportunity allows you to make a lasting impact by opening your home to an adult with an intellectual and/or developmental disability (I/DD) who is in need of safe, stable housing and daily support.

As an AFL Provider, you will provide more than care—you will provide a home. AFL Providers welcome an individual into their residence and create a supportive, family-style environment where they can live safely, build independence, develop life skills, and become active members of their community. If you have a passion for helping others and want to change someone's life while earning competitive compensation, we encourage you to apply.

Responsibilities

  • Provide a safe, clean, stable, and welcoming home environment for an individual receiving Assistant Family Living (AFL) services.

  • Offer housing by welcoming an individual into your home and supporting them in a family-style living setting.

  • Assist with activities of daily living (ADLs), including personal hygiene, dressing, grooming, meal preparation, medication reminders, and household routines.

  • Teach and encourage independent living skills such as cooking, cleaning, laundry, budgeting, shopping, and home management.

  • Support individuals in achieving goals outlined in their Person-Centered Plan (PCP) and Individual Support Plan (ISP).

  • Encourage community inclusion by assisting with recreational activities, appointments, employment, volunteer opportunities, church, and social events.

  • Provide emotional support, mentorship, encouragement, and positive reinforcement.

  • Monitor and document daily progress, behaviors, health concerns, and significant events in accordance with agency policies.

  • Collaborate with Qualified Professionals (QPs), healthcare providers, guardians, family members, and CareLink Solutions staff to ensure quality care.

  • Transport individuals safely to appointments, community activities, and other approved destinations as needed.

  • Respond appropriately to emergencies or behavioral situations while following agency procedures and safety protocols.

  • Maintain confidentiality while protecting the rights, dignity, and independence of each individual served.

Qualifications

  • High school diploma or GED required.

  • Previous experience working with individuals with intellectual and developmental disabilities, behavioral health, direct support, healthcare, or human services is preferred but not required.

  • Compassionate, patient, dependable, and committed to providing person-centered care.

  • Excellent communication and interpersonal skills.

  • Ability to maintain accurate documentation and complete required reports on time.

  • Valid driver's license, reliable transportation, and acceptable driving record.

  • Ability to pass a criminal background check and drug screening.

  • CPR/First Aid certification or willingness to obtain upon hire.

  • Ability to meet state and agency requirements for providing AFL services.

  • Must have adequate living space within your home to accommodate an individual receiving services.

What We Offer

  • Competitive compensation

  • Ongoing training and professional development

  • 24/7 support from experienced Qualified Professionals and agency staff

  • Flexible and meaningful work

  • Opportunity to make a lasting difference in someone's life

  • A supportive team dedicated to your success as an AFL Provider
