Job Summary :
We are seeking a highly motivated and experienced Director of Social Services to join our team at Unity Health and Rehabilitation Center. As a Director of Social Services, you will be responsible for overseeing the social services department and ensuring that our residents receive exceptional care and support. You will work closely with our nursing staff, therapists, and other healthcare professionals to develop and implement comprehensive care plans that address the unique needs of each resident.
Responsibilities :
Manage and lead the social services department, including supervising social workers, activity therapists, and other support staff
Develop and implement comprehensive care plans that address the social, emotional, and psychological needs of our residents
Collaborate with healthcare professionals to identify and address any social or emotional concerns affecting resident care
Provide education and support to residents and their families on resources and services available in the community
Develop and implement activities and programs designed to promote resident engagement, socialization, and overall well-being
Ensure compliance with all regulatory requirements and standards related to social services
Develop and maintain relationships with community organizations and resources to ensure access to services and support
Requirements :
Master's degree in Social Work or related field
Minimum 2 years of experience in a social services leadership role
Strong knowledge of social services principles, practices, and regulations
Excellent communication, interpersonal, and leadership skills
Ability to work effectively in a fast-paced environment
Strong analytical and problem-solving skills
