Schedule / Job Type

Contract | 13-Week Travel Assignment | Days, 8:00 AM - 4:30 PM | Weekend rotation included | Monday-Friday and Friday-Monday shift options available
Key Responsibilities

Manage a caseload of approximately 1:20 patients as an RN Case Manager across inpatient units of approximately 20 beds each
Conduct discharge planning assessments and coordinate comprehensive discharge planning for assigned patients
Collaborate with a multidisciplinary team including social workers and other case management staff to support patient transitions
Utilize electronic medical record systems (Meditech and CarePort) and mobile technology to document and coordinate care
Adapt effectively across different units as patient and staffing needs require

Required Qualifications

Active RN license required
Minimum 1 year of current Case Management RN experience
Strong communication skills and professional, collaborative approach
Ability to adapt to different unit environments and patient populations

Preferred Experience / Skills

Experience with discharge planning in an acute care setting
Emergency Department (ED) experience
Palliative care knowledge and goals-of-care conversation experience
