Description Clinical Pharmacist at Providence Santa Rosa Memorial Hospital in Santa Rosa, CA. This position is per diem and will work 10-hour, Day shifts. The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Required Qualifications : PharmD - Pharmacy Doctorate or Bachelor's Degree in Pharmacy California Pharmacist Registered upon hire Basic Life Support (BLS) upon hire - where applicable Preferred Qualifications : Acute care hospital pharmacy experience, 1 year or equivalent experience.
